2011 Service Highlights
Crossroad provides accessible, affordable, community-based health care and mental health counseling to the residents of Greater Cincinnati. We have become an increasingly important community resource, providing more than 20,000 visits annually for more than 6,600 patients who otherwise would not be able to afford care.
Who We Serve
- Provided 20,020 visits for 6,626 patients.
- 52% of patients are African American, 28% are Caucasian and 11% are Hispanic/Latino.
- 92% of patients have incomes at or below 200% of the Federal Poverty Level (annual income of $44,700 or less for a family of four).
- 26% of all patients are uninsured; 45% of adult patients are uninsured.
Improving Mental Health
- 25% of our adult patients have a diagnosable mental illness.
- We provided 363 visits for mental health counseling.
- We expanded our mental health services by adding a part-time psychiatrist, Dr. Bryan Cairns, to our staff.
Focus on Children
- 4,017 patients are children.
- 85% of our pediatric patients at 24 months of age were fully immunized. The state average for Ohio is 71%.
- We participate in WeTHRIVE! and Healthy Kids Ohio & The Ounce of Prevention Project, two childhood obesity collaboratives. Pediatric patients are screened for weight issues; parents receive counseling and education on nutrition, physical activity and community resources.
- Crossroad Pediatrics West grew in 2011, reaching capacity at 1,540 patients. We plan to move into a larger office space and hired Dr. Cyndi DeLaat to allow for further growth on the West Side.
- 4% low birth weight rate for newborns. The Ohio average for community health centers is 9.1%. The City of Cincinnati average is 10.7%.
Adult Health Care
- We achieved a 62% PAP smear rate for female patients – well above the 46% state average for community health centers serving patients with low income.
- 74% of pregnant patients access pre-natal care in the first trimester of pregnancy. The average for Ohio community health centers is 69.9%.
Implementing Best Practices
- We continued implementing Electronic Medical Records (EMR) in 2011 to provide best practice medicine to patients. EMR was fully functional in January 2012.
- We entered into the second year of the Beacon Project, a two-year process to become a nationally certified “Patient Center Medical Home,” which will help Crossroad improve patient care.
- We began a collaboration with Buckeye Community Health Plan to provide a case manager on site to help high risk patients enrolled with Buckeye, manage severe, chronic illness..
